eVerifile Applicant Consent Form for Background Investigations

  • Prior to ordering a background report, employers must (i) provide a disclosure in a separate document from the employment application to the applicant/employee that a consumer report is being ordered, and (ii) obtain a written authorization from the applicant/employee.  State law may also impose additional requirements.

FCRA Applicant Summary of Rights

  • This document must be provided to any applicant before taking Adverse Action against them based in whole or in part on a consumer report such as a background investigation.  Adverse Action is considered as any denial of benefits, such as denial of job, promotion, or dismissal, etc.  The Summary of Rights outlines the major rights of consumers under the FCRA.  State laws may also impose additional requirements.
